I understand that Nahunta Hall has student behavior policies in place, including a Safe Schools Policy. My student will comply with all such policies and procedures. I understand that Nahunta Hall has a zero-tolerance policy for bullying and harassment. Disrespectful words or actions are unacceptable, particularly those focused on race, ethnicity, sexual orientation, gender, religion, social class, appearance, or disability, and will result in severe administrative action as allowed by school policy and State law.

I hereby give Nahunta Hall permission to use images of my child taken at school or school approved events for any purposes related to Nahunta Hall. I understand that these images may be used without any further notification. I release Nahunta Hall and its governing board, employees, agents, representatives or assigns from all liability which may arise from any and all claims or demands made by me or any third party ensuing from or in connection with my child’s participation, including any and all claims for libel and invasion of privacy.

I have read the Utah Health Department regulations and Nahunta Hall policies regarding student illnesses (available in the online Family Handbook) and I agree to keep my child home when they are ill. I agree to notify Nahunta Hall of any food allergies and health concerns that may limit participation in school activities. Students may be required to wear a mask pending local health guidelines and requirements.
